Everyone close to the caster, if not higher level than the caster, must resist or die.
From the book
The caster speaks the word, and every person or creature within 20 ft. has to save vs Will or die on the spot. Anyone of a higher level of ability than the caster is immune, and those who make the save take no harm at all. The spell is Non-variable.

A scene
On the Isle of Demons, the Hantu came swarming out of the sulphur mist at the wizard Aurelian, a dozen of them, clawed, scaled, winged, no two alike. He waited until they were almost on him, then spoke the word. Most of the Hantu dropped dead where they stood, some mid-leap. Two of the lesser ones fought off the word and kept coming, and one great horned brute, older and stronger than Aurelian himself, never even felt it and strode forward. Aurelian drew his sword and backed toward the boats.
