From the book

The caster conjures an illusion so horrifying that everyone who sees it, within 100 ft., must save vs Perception or be driven permanently insane. A scene

The adventurers had fought their way up to the top floor of the Citadel of Tok, where one of the Twelve Masters was waiting for them. He did not raise a hand. He simply showed them something, conjured in the air of the chamber for no longer than a minute, and nobody who saw it has ever managed to describe it. Two of the adventurers turned away in time and kept their minds. The other three still wander the streets of Tartessos today, muttering about spirits in the walls and things with too many mouths.
